We at Meridian Internal Medicine, P.A. are pleased you have chosen us for your health care needs.  There are a few things to review to make sure your first visit is as productive as possible.

Please go to the "forms" page to download the necessary paperwork. Bring this completed paperwork to your first appointment. If you are unable to download the paperwork, then arrive 15 to 30 minutes prior to your appointment to complete the forms. Although paperwork is inconvenient, it allows Dr. Prochnau and her staff to gather general information so that your time with them can be spent focusing on the main reason for your visit.

Every effort is made to see you at your scheduled time. Timely arrival to scheduled appointments will assure that you are seen and help ensure that our practitioners stay on schedule.

Bring ALL prescription medications that you are taking so we can review them. Also, bring your insurance cards and driver's license to the appointment so that the most accurate information can be obtained for filing your insurance.

Dr. Prochnau is a preferred (in-network) provider for most major insurance companies and will be happy to file your insurance claim for you.

We accept fee for service.  Please call for fee for service rates.

As plans change frequently, you should contact your insurance company to make sure Dr. Prochnau is an in-network provider and that the services of her practice are covered before your first visit.
